The following are requirements to volunteer at the U.S. Space & Rocket Center:

- Minimum 16 years of age.

- Reside within a 40 mile drive of the Center.

- Commit to volunteering an average of 6 hours per month.

- Successfully complete a background check.

- Attend a "Volunteer Orientation".

- Pay one-time administrative fee of $35.

Commitment

Volunteers are an important part of the U.S. Space & Rocket Center family. We ask you to establish a set schedule, be it once a week, ever other week or once a month. We are looking for volunteers to on average volunteer 6 hours each month. Service hours will be evaluated semi-annually.

If accepted to volunteer, I commit to volunteering on average 6 hours per month or 72 hours over the course of one year.

I certify that all the information given by me on this application or in supplemental form is true and correct to the best of my knowledge and belief. I further understand that false or misleading statements or consequential omissions of any kind on this application or supplemental forms are sufficient cause for my not being selected as a volunteer.

I agree, understand and authorize the U.S. Space & Rocket Center or its agents to investigate my background. I authorize the persons or organizations referenced in this application to give the U.S. Space & Rocket Center any and all information they might have personal or otherwise with regard to any of the content covered by this application and I release all such parties from all liability for any damage that may result from furnishing such information.

I also agree and understand that this investigation may include an investigation regarding my character, general reputation, and personal characteristics. If any such investigation results in denial of becoming a volunteer, I shall be so advised. In these instances, the U.S. Space & Rocket Center shall supply the investigative report, and I will be given an opportunity to correct any misinformation contained in any such report.

If accepted in to U.S. Space & Rocket Center Volunteer program I do hereby, voluntarily execute the following:

USSRC VOLUNTEER WAIVER, RELEASE, HOLD HARMLESS AND INDEMNIFICATION AGREEMENT

I have agreed to serve as a volunteer for the U. S. Space & Rocket Center in Huntsville, Alabama and I recognize that my volunteer participation is a privilege afforded to me by the USSRC. I fully understand, appreciate and assume all the risks associated with my volunteer duties. In exchange for my participation, I hereby agree to the following:

1. I voluntarily waive, release and hold harmless the USSRC, its elected and appointed officials, officers, employees, agents and other volunteers from any and all claims, causes of action and damages for bodily injury or death that I may suffer as a result of, or in any manner connected with, directly or indirectly, my participation as a USSRC volunteer when such bodily injury or death is the result of my own negligent or intentional acts or omissions or those of another volunteer. I understand that this waiver and release precludes my right to recovery of damages in the event I am injured in the course of performing my volunteer duties.

2. I shall defend, hold harmless and indemnify the USSRC, its elected and appointed officials, officers, employees, agents and other volunteers, from and against all damages, claims, liabilities, causes of action, judgments, settlements, costs and expenses (including, but not limited to, reasonable expert witness and attorney fees) that may at any time arise or be claimed by any person as a result of bodily injury, death or property damage, or as a result of any other claim or cause of action of any nature whatsoever, arising from or in any manner connected with, directly or indirectly, my negligent or intentional acts or omissions in performing my volunteer duties for the USSRC as a volunteer.
