Parent Consent For Minor

I understand that by completing a volunteer application it does not guarantee a volunteer assignment will be offered to my child, nor is it an offer of paid employment of any kind with the Fleet Science Center.

By submitting this form I am permitting my child to volunteer without getting pay and agreeing that I will not hold the Fleet Science Center liable in the event of my child’s injury or accident. I understand that if it becomes necessary to obtain emergency medical care for my child, such care may be obtained without further consent of a guardian. Additionally, I will personally assume responsibility of any costs of such care and/or transportation to receive that care which is above and beyond what is covered by the Science Center’s insurance.

We also want to make you aware of our requirements. We ask that Fleet Volunteers commit to a minimum of 80 hours. No letters of recommendation or verification of hours will be administered until these requirements are met. If your son/daughter needs verification for school, please plan their volunteer time accordingly.


The Fleet Science Center:
We will invest the resources necessary to keep you motivated and engaged, including training, events, discounts, and support.
Will provide a reference if needed after 80 hours of service are completed.
Will address any issues that arise and provide reasonable and appropriate support for you.
Will provide safe working conditions with the right supervision, training, and tools for the tasks assigned. 
Will communicate with all volunteers keeping everyone up to date with museum developments. 
To ensure the safety of our employees, volunteers and guests we require all volunteers to submit to a background check. Specifically, we will check for criminal and sexual offense records in the last seven years. In addition, we will confirm the validity of your social security number.
Volunteer Applicant:
I understand that the Museum has policies and procedures in the volunteer handbook that will be given to me and is also available via Volgistics or through any department. I understand that if I do not follow these policies and procedures, or act in any way incompatible with the image or acceptable conduct of the Museum, I will not be able to continue as a volunteer. 
I will make every attempt to volunteer for the shifts I have selected. If I am unable to attend, I will follow the cancelation procedures defined in the handbook. I understand that by not following these policies, I may not be able to continue volunteering.
I understand that the Fleet is not responsible for loss or damage of personal property. I am responsible for storing my belongings at the Fleet in a secure area if needed, but discouraged from bringing any valuables to my volunteer shifts.
I understand that I am a volunteer and will not be paid for my time at the Fleet in a volunteer assignment.
I understand that if I have access to a computer while volunteering at the Fleet, it is for business related use.

I understand that as part of my volunteer role, I may have access to confidential information or personnel files, and should maintain the highest discretion by not disclosing personal and private information.

I understand that by allowing my photograph to be taken at the Fleet, I am authorizing the irrevocable right to use my photograph for publicity, social media, and any other lawful purpose without compensation. I also understand that it is my responsibility to advise someone in advance, not to take my photograph if I do not wish to be photographed.

I am not allowed to drive the Fleet vehicle, but can ride as a passenger understanding that I release the Fleet from any liability incurred.

I understand that volunteer placement is a selection process and not all applicants are accepted into the program. I also understand that the Fleet Science Center reserves the right to place volunteers in the area that staff feels is best suited to both their skills and the needs of the Fleet Science Center.

The processing of background checks is regulated by the Fair Credit Reporting Act which is a federal and state law that strictly outlines what information we are and are not able to collect. Please be assured that we ARE NOT checking your credit and there will be no record of this background check on your credit report. Background checks will be run every three years for current Fleet volunteers. As with the initial background check, if a repeat background check reveals criminal activity which the Fleet finds unacceptable, the employee or volunteer could be subject to termination.
The Fleet is committed to providing a safe place for all visitors including children, youth, who participate in the many programs and educational activities sponsored by our organization. Considering the recent trend of sexual abuse around the nation, it is necessary for us to add the following commitment statement as part of your orientation process. The following policy statements reflect our commitment to preserving the Fleet as a safe and inviting environment for all who would enter and as a place in which all people can experience science through relationships with others. Please read and understand each one before signing.
• No adult who has been convicted of child abuse (either sexual abuse, physical abuse, or emotional abuse) shall work with children or youth in any organizational activity.
• All employees/volunteers involved with children or youth at the Fleet must have read, agreed and signed all policies and procedures and commit to upholding these standards
• All adults involved with children and youth of the Fleet shall abide by the Child Protection Policy at all times.

• The Fleet maintains a “zero tolerance” policy. Any staff member found in opposition to this Fleet policy will be terminated immediately.

• All adults involved with children and youth of the Fleet shall attend regular training and educational events provided by this organization to keep employees/volunteers informed of all policies and laws regarding child abuse.
• All adults involved with children and youth of the Fleet shall immediately report to their supervisor any behavior that seems abusive or inappropriate between: peer to peer, staff to child, and staff to staff.
• You shall be advised that ALL reporting (by law) is kept confidential and will in no way harm your position with our organization unless false allegations are found to be made.
• You shall be advised that child sexual abuse is punishable by law and the Fleet is bound by law to report allegations of sexual abuse to the proper authorities.

We also expect the employees of client companies, vendors, visitors and others to conduct themselves in a manner that does not create an offensive, intimidating or hostile working environment for our employees and volunteers.
