Demographic Information

You may optionally provide the following information. It is used only to help us get a better idea of the demographic make-up of our volunteers.

Availability

Please indicate the days and times where you would be able to commit to a volunteer shift.

PLEASE NOTE: Availability requirements differ between departments. In the Wildlife Hospital, volunteers commit to 1 set shift. For example, the Monday morning 9am-1pm shift could be your permanent shift. We require reliability of our volunteers, as they are our staff.

Medical Information

Do you have any medical conditions that we should be aware of (severe asthma, herniated discs, heart conditions, allergies, etc.)?

Physical Limitations or Special Needs

Do you have any physical limitations or special needs that we should be aware of?

Tetanus Vaccination (REQUIRED)

Please enter the year of your last tetanus shot. A current tetanus vaccination is REQUIRED to volunteer in any capacity. If you do not enter the year of your last tetanus shot, we are unable to process your application.

Rabies Vaccinations

Have you had any rabies vaccinations? If you have, enter the year of your last vaccination or titer (titer required every 2 years). **Rabies vaccinations are not a standard vaccination. They would only be administered in the case of exposure to a rabies vector species or pre-exposure vaccination for a position at an animal care facility or foreign travel. Rabies vaccinations are not required to volunteer in the Wildlife Hospital, but you MUST receive rabies vaccinations to work with rabies vector species at Cedar Run (fox, raccoon, skunk, bats, and groundhogs).

Email & Text Preferences

We like to keep volunteers informed of important news, schedules, and volunteer opportunities by email, however will not send you any email you prefer not to receive. Use the checkboxes below to select the kinds of email you would like to receive from us.

PLEASE NOTE: Volunteer Application Fee

All volunteers will be asked to pay a one-time non-refundable application fee. This fee covers the cost of a criminal background check and supports staff time and resources associated with training and processing. This is a criminal history only check, not a credit check. Our insurance mandates this check because we have groups of children on the premises year round. Unfortunately, we cannot accept background checks provided by applicants - they must be processed through our system.

Volunteers UNDER 18 will be asked to pay $20, and do not require a criminal background check.

Volunteers 18 AND OVER will be asked to pay $40, and will be sent instructions on completing a criminal background check after their application has been reviewed. This is a criminal history only check, not a credit check.

APPLICANTS OF ALL AGES WILL BE SENT INSTRUCTIONS FOR SUBMITTING THE VOLUNTEER APPLICATION FEE ONCE THEIR APPLICATION HAS BEEN REVIEWED.

Volunteers under 18 Years of Age

If you are under 18 years of age, you must print a Volunteer Release Form that must be sign by your parent/legal guardian. A printable form will be sent to you by our Volunteer Coordinator when your application has been reviewed.
