Statement of Certification

I certify that I have answered truthfully and have not knowingly withheld any information relative to my application.  I agree that I offer my services as a volunteer with no expectation of monetary compensation.

Attending Teen Climate Communicators in-person and virtual meetings means you agree to the Release of Liability, Virtual Etiquette Release, and Photo and Video Release.

Release of Liability:

I have requested the Anchorage Museum Association to allow my child to participate in Teen Climate Communicators and related activities. As a condition of receiving this benefit, I, the undersigned, do hereby agree to the following:

I understand that participation in Teen Climate Communicators activities can expose my child to dangers both from known and unanticipated risks.  Acknowledging that such risks exist, I hereby release and discharge Anchorage Museum Association, its officers, agents, and employees from any and all claims or liability for personal injury or property damage my child may suffer while participating in the activity; including, but not limited to, any claim arising out of any condition of the premises at which the activity is held or the conduct of any person in connection with the preparation for, supervision of, or conduct of any activity, whether planned or unplanned.  I specifically agree to release and hereby release Anchorage Museum Association and the officers, agents, and employees of the Teen Climate Communicators for any negligence, or its officers, agents or employees.

Virtual Etiquette Release:

I understand that my child’s participation in the virtual Teen Climate Communicators is voluntary and that I retain full supervisory responsibility for any and all activities. Please note that the museum will provide login instructions for the virtual meetings, but is not responsible for technical support due to, but not limited to, loss of internet at your home, personal equipment malfunctions including web camera, computer microphone, and computer, or errors due to antivirus software on your computer/tablet. Appropriate behavior for all participants will be expected during all virtual meetings and activities. Bullying, including cyberbullying, during a session or outside of a session will not be tolerated. I will ensure that my child is appropriately dressed and that the computer and camera that they are using does not show inappropriate things in our home.  If staff notice something on your child’s video that staff determines is not appropriate, they will turn off your child’s video until the issue is resolved. The Anchorage Museum reserves the right, upon notification of parents, to dismiss any student during any sessions for conduct that is deemed detrimental to the well-being of other students, staff, or to the overall operation of the virtual meetings. 

Photo and Video Release: 

I hereby authorize the Anchorage Museum Association to photograph or videotape my child and agree that the AMA may indefinitely use or permit other persons to use the negatives, prints, or videotape prepared for such purposes and in such manner as may be deemed necessary to promote the Anchorage Museum. Virtual meetings may be recorded for educational and safety purposes and may be posted online. If I do not wish for my child to be in the recording, I should immediately turn off the camera and mic. Leaving either of those two on is implied consent. It is solely my responsibility to take any necessary steps to prevent my child being recorded.

I certify that I have read the above provisions of this form, I understand them, and I agree to be legally bound by them.
