Thank you for your interest in the volunteer program at Sunrise Hospital & Medical Center and Sunrise Childrens Hospital. We consider volunteers a vital part of our hospital providing priceless contributions to the well being of our patients, their families, and our staff.

The following are the steps required to become a volunteer:

1. Please complete this online application.

2. Once you have submitted your completed application, you will be contacted by our Volunteer Supervisor and invited to participate in a group meeting that will be conducted at the hospital to further discuss our program opportunities and your possible role in our program. We do not place volunteers for internship, shadowing, or observing positions.

3. To ensure the safety and security of our patients, acceptance into our volunteer program is contingent upon passing a background check, drug test, immunizations check, presenting proof of COVID vaccination and tuberculosis (TB) test. During flu season (September to March), volunteers must obtain a flu vaccination. If you choose not to be vaccinated, you will be required to wear a mask at all times while volunteering at the hospital during flu season.

4. Once you have been cleared of all on-boarding requirements, you will need to attend a 4-hour Volunteer Orientation. During this orientation, you will be educated on Sunrise Hospital mission and values, safety and security protocol, diversity and abuse, ethics and compliance, volunteer policies and procedures, as well as receive necessary hospital training in customer service, HIPPA, infection control (PPE), wheelchair safety, and non-clinical comfort CARE.

5. After you have completed orientation, you will then be scheduled for department specific training. Once this training is completed, you will be placed on a schedule.

Your commitment to your schedule is critical to the overall volunteer program integrity as well as obtaining a positive and productive hospital experience.

Due to the timing necessary for all health and safety processing documentation required as well as completion of orientation and department specific training, becoming a volunteer may take up to 4-6 weeks to complete.

Time Commitment

Volunteers are asked to commit to volunteering for a MINIMUM of 100 hours over a continuous 6 month period. Please consider your schedule, school, work, family, etc. before making the commitment to volunteer. Any written documentation of service hours, personal recommendations, or proprietary testing information will NOT be released until you achieve a total commitment of 100 service hours.


Please indicate all days and times you are available to volunteer. You will ONLY be scheduled for one 4-hour shift per week. Although we do our best to accommodate your schedule, final shift placement will be determined based on hospital department needs and openings we have available.

Pre-Screening Disclosure

To comply with hospital safety and security protocol, all volunteers must have a background check. Please answer the 2 pre-screening disclosure questions below. (A conviction will not necessarily disqualify an applicant from volunteering.)

1. Regardless of when it occurred, have you ever been convicted of ANY law violation, including misdemeanors? Include any plea of “guilty” or “no contest”. Exclude minor traffic violations. (Arrest or charges that have been expunged need not be disclosed). Please indicate YES or NO in the space provided below. If YES, also provide date, place, and nature of each conviction.

2. Are you currently excluded, suspended, debarred, or otherwise ineligible to participate in Federal or State Health Care Programs including Medicare and Medicaid, or have you been convicted of a criminal offense related to the provision of health care items or services but not yet been excluded, debarred, or otherwise declared ineligible? Please indicate YES or NO in the space below.

Professional Reference

Please provide one reference which will allow us to better assess your ability to fulfill the responsibilities involved in our volunteer service program. All information is confidential. Reference should be someone outside your immediate family such as a boss/co-worker, mentor/counselor, teacher/professor, etc.

In Case of Emergency

I understand that Sunrise Hospital will not provide me with compensation, unemployment insurance, worker’s compensation, or any other benefit of employment in connection with my volunteer service and that my personal health insurance will be expected to respond on a primary basis for any accident or injury that I may sustain while functioning as a volunteer. I agree to notify the Guest Services Department and Sunrise Hospital Public Safety along with completing the proper accident paperwork of any such accident or injury. In case of emergency, please indicate who you would like us to contact on your behalf.

Volunteer Code of Ethics

During your time at Sunrise Hospital & Sunrise Medical Center, we expect you to behave professionally and be respectful. You represent our hospital and your choices must reflect and send this message to our patients, their families, and our staff.

1. Show up on-time for your shift on the same day and time each week. You may not come and go as you please. Our staff will depend on you and your consistency is important. You are a key part of the overall patient experience and must take your roll seriously.

2. Come dressed in your proper uniform that consists of a polo shirt (provided) along with pants in either black, navy, or beige (khaki). Volunteers may NOT wear denim jeans, shorts, Capri pants, leggings/stretch pants, hats, sleeveless shirts, or sandals.

3. Volunteers are required to wear a hospital volunteer photo ID badge at all times, which will be issued to you on your first day of training. This badge is the property of the Sunrise Hospital and must be returned when you complete your volunteer service.

4. For safety and security purposes, you must sign in and out at the front desk EVERY time you are at the hospital. This is also how we track your volunteer service hours.

5. If you are unable to make your shift, you must notify Volunteer Guest Service as instructed. If you miss two shifts without proper notification, you will be removed from the schedule.

6. Cell phones may be with you while you’re volunteering, but they must remain in silent or vibrate mode. Volunteers are asked to only accept incoming calls and make outgoing calls during breaks, not while on duty.

7. Please do not smoke or chew gum while on duty.

Reasons for Termination

A volunteer may be terminated for the following reasons:

1. Breach of confidentiality.
2. Inappropriate behavior.
3. Not adhering to uniform dress code requirements.
4. Absence without proper notification to Volunteer Guest Services.
