By submitting this online form, I acknowledge that I have read and understand this application and agreement as stated below, as well as the policies and procedures detailed in the Volunteer & Intern Handbook. I hereby release BAM, its employees, agents, representatives, and volunteers from any or all liability that may arise in connection with the information shared in a reference or recommendation letter. I understand that either the Museum or I may end our relationship without cause or notice. These arrangements cannot be changed except in writing, signed by the department Manager.
As part of the screening process, we are required to conduct background checks on applicants through the Washington State Patrol. If you would like your record sent to you after the background check has been completed, you may email the Manager of Volunteer Programs at email@example.com to request a copy.
By answering the questions, you indicate your willingness to have an inquiry made to the Washington State Patrol. Decisions regarding volunteer background checks are made on an individual basis.
ASSUMPTION OF RISK & RELEASE OF LIABILITY
I understand that my participation in the Bellevue Arts Museum’s programs, operations, and/or maintenance is a voluntary activity and that I am donating my time and my labor by my own free choice. In consideration of permission to participate at Bellevue Arts Museum, I hereby agree to assume the risks of property damage, injury, illness, or death in any way associated with my participation in this activity. I agree to release, defend, indemnify, and hold harmless Bellevue Arts Museum, its employees, agents, representatives, and volunteers from any and all liability that may arise in connection with this activity. I agree that the terms stated herein shall serve as an assumption of risk and release of liability for my heirs, estate, executor, administrator, assignees, and for all members of my family. Furthermore, I give my permission to have photos/video recordings taken and/or my name printed and used for publicity purposes, without recompense during Bellevue Arts Museum volunteer activities.
By submitting this form, I acknowledge that I have carefully read this ASSUMPTION OF RISK & RELEASE OF LIABILITY and fully understand that I am waiving any right that I may now or hereafter have to bring a legal action to assert any claim against Bellevue Arts Museum in connection with my participation in this volunteer activity.
I understand and agree that submitting this application form does not automatically register me as a Bellevue Arts Museum volunteer. By submitting this form, I agree, to the best of my knowledge, that the information is true and complete. I understand that falsification of this application is grounds for dismissal as a volunteer, and possible further action.
Bellevue Arts Museum staff members work with confidential information concerning donors, organizations, and artists. Volunteers may also end up handling confidential information during their experience at BAM. To maintain the privacy and integrity of BAM records, all volunteers are required to preserve the confidentiality of any personal or financial information.
I understand that as a Bellevue Arts Museum volunteer, I am bound by the confidential nature of any information I may learn about donors, supporters, members, and artists. I agree only to discuss or share confidential financial or personal information with Bellevue Arts Museum staff relevant to the projects and context of my volunteer work. I will not reveal any of this confidential financial or personal information to anyone outside of the Bellevue Arts Museum staff.
CONDUCT WITH MINORS POLICY
Bellevue Arts Museum (BAM) is dedicated to providing patrons with a safe and secure environment to foster and encourage creativity. The Museum recognizes that persons under the age of eighteen are an especially vulnerable group and that misconduct with a minor is harmful to the minor, his or her family, BAM, and society. Therefore, all persons associated with BAM, including but not limited to employees, independent contractors, and volunteers, shall have no one-to-one contact with a minor on or off the Museum premises. Any contact with a minor shall take place in a public area.
1) To create a safe and secure environment for all.
2) To define and provide notice to, but not limited to, Museum staff, independent contractors, volunteers, and all other persons associated with Bellevue Arts Museum that individuals who violate this policy will be held accountable for acts of unprofessional conduct.
3) To assure the community that Bellevue Arts Museum implements clear measures to ensure the safety of minors associated with the Museum.
Reporting a Problem
If you believe this policy has been violated, we ask you to immediately and confidentially report such violation to the Managing Director who, with any expert assistance deemed necessary, shall undertake a thorough investigation and, if the policy or the law has been violated, deal appropriately with the violation.
If you believe a situation is or may be developing which, if not resolved, could lead you into violating this policy or create an appearance of violating the policy, we encourage you to confidentially discuss the potential problem and resolution with the Managing Director.
Bellevue Arts Museum adheres to the highest ethical standards. Anyone who commits an act of unethical conduct proscribed within this statement will be held accountable for such conduct.
Process for Screening Minors
As part of the screening process, Bellevue Arts Museum is required to conduct background checks on all Museum employees, independent contractors, volunteer applicants, and anyone who works on behalf of the Museum through the Washington State Patrol. Individuals may also be subject to a Federal Background check.
Bellevue Arts Museum believes the above policy is in the best interest of the Museum and its patrons, volunteers, employees, independent contractors, and all other persons associated with the Museum. We greatly appreciate your participation and cooperation in creating a safe and secure environment for all.
By agreeing, a parent or legal guardian of the minor, if applicable, has read and agreed to these terms.
