The information below is used to help us understand the demographic make-up of our volunteers and to meet background check and minimum requirement standards. Only City Human Resources staff are allowed to see this information.

For example:
- Date of Birth is required because most of our assignments have age restrictions. Our coordinators can see your age, but not your full date of birth.
- Driver's License Number is required for us to perform background checks on volunteers in assignments with driving duties.

Youth Volunteer Policy

Volunteers under the age of 18 years are required to provide parental/ guardian permission and consent to be assigned and participate as a volunteer for the City of Springfield. You do not need to submit the permission form at this time. The volunteer coordinator will contact you for a copy of the consent form at the time you are assigned to a position.

It is the responsibility of the group leader to collect and maintain Parental Consent forms for all participants under the age of 18 years. These may be the group's own forms or the group leader may contact the Volunteer Office to obtain Parental Consent forms that they may use. A form must be provided to the city coordinator before youth volunteers will be allowed to participate.

All youth participants under the age of 18 years are required to be accompanied by an adult, unless assigned to a volunteer program with City supervision. Youth over 18 may be permitted to work without adult accompaniment, based on the nature and scope of the activity and the ability of the park to provide adequate supervision for the youth volunteer. Groups must have adequate adult supervision of youth participants.

General Volunteer Policies

- Volunteers perform service without compensation and are not considered employees of the City of Springfield.
- The City does not provide Worker’s Compensation or any other insurance coverage for volunteers.
- The City cannot guarantee volunteer placement.
- Submission of a Volunteer Application does not imply automatic acceptance as a City volunteer. There may be additional required qualifications depending on the nature and scope of volunteer assignments.
- Background checks are required of volunteers in assignments involving children or vulnerable populations; driving on the City's behalf; collection or handling of revenues; the discharge or handling of weapons, or firearms.
- Acceptance as an individual volunteer is contingent upon successful completion at all levels of screening.
- The City reserves the right to disallow volunteer candidates for any reason which the City, in its sole judgment, determines may affect the best interests of the City. The City reserves the right to withhold the reason(s) for such refusal.
- The City accepts the service of all volunteers with the understanding that such service is at the sole discretion of the City. Volunteers agree that the City and the Volunteer may at any time, for any reason, decide to terminate the volunteer’s relationship with the City. Notice of such a decision should be communicated as soon as possible to the volunteer’s supervisor.

- Group Volunteers must identify a Group Leader who will be responsible for the safety and well-being of all group members. The group contact agrees to:
1. Provide coordination and supervision for group while volunteering.
2. Ensure all members adhere to applicable rules, regulations, and procedures.
3. Report to The City any behavior, actions, or language inappropriate for the established activity, location, or group which may endanger the health and/or well-being of the individual, other group members, or visitors.

As a volunteer to the best of my knowledge. I understand that false or misleading statements or missing information is cause for rejection of application, removal of name from eligible list, or dismissal from the program. I hereby waive my rights to claims or damages against any employer and the City of Springfield, its officers, agents, and employees, in regard to this exchange of information concerning my background. I hereby authorize and direct you to permit the City of Springfield and or the Springfield Police Department to review my criminal records and Department of Motor Vehicles (DMV) records. I recognize that I must review the position assignment description before beginning service and alert the volunteer coordinator should I not meet the minimum requirements. I acknowledge that I will be enrolled in the Oregon State driving record Automated Reporting System (ARS) if my duties involve driving on city business. I also authorize to permit any materials listed above to be copied and retained by the City of Springfield and or the Springfield Police Department. I authorize the use of my image, likeness, video or audio recording for non-profit use.

I understand that nothing in this application or subsequent assignments creates an employment relationship with the City of Springfield. I will defend, indemnify and hold harmless the City of Springfield, its officers, employees, and agents from and against all liability or loss and against any and all claims, actions, causes of actions, proceedings or appeals based upon or arising out of or arising from or in connection with my conduct or performance with the City of Springfield including but not limited damage or injury to persons or property and including without limitation attorney fees and expenses; except for losses, claims or actions resulting from the sole negligence of the City of Springfield.

I understand that some assignments may be considered risky or dangerous and I have the right to refuse any assignment or task.

I understand that the City of Springfield does not provide Workers' Compensation or any other insurance coverage for volunteers.
