WHS Waiver and Agreement

I would like to participate in the volunteer program at Willamette Humane Society (WHS). I am doing so entirely of my own initiative, risk and responsibility, and with the understanding that if I work in direct contact with animals, there are inherent potential dangers, including but not limited to, zoonotic disease and animals bites and scratches.

I certify that the information I have provided in this questionnaire is accurate and true. I give permission for this information to be verified and understand that any false information may nullify my volunteer service with Willamette Humane Society. I understand that WHS reserves the right to conduct a background check.

I understand that I am responsible for reading the WHS Volunteer Handbook and abiding by its contents.

As a Willamette Humane Society Volunteer, I further agree to:

• Demonstrate respect, kindness, and compassion to shelter cats, shelter dogs, my fellow volunteers, staff, and the public.

• Understand and observe shelter protocols, including all kennel signage, with the understanding that these protocols are in place for the health and safety of people and pets.

• Work cooperatively with fellow volunteers, staff, and the public.

• Offer constructive feedback to others in a kind and respectful manner, giving others the benefit of the doubt. Accept constructive feedback from fellow volunteers and staff, seeking clarification when needed.

• Participate in volunteer meetings and training. Ask for, and receive, training in any new area before engaging in any tasks.

• Follow all procedures and instructions for my volunteer role, including protocols for specific animals. Seek clarification if I am unsure about a protocol.

• Communicate ideas, suggestions and concerns to my supervisor, the Volunteer Team, or any member of the leadership team.

• Follow instructions provided by my supervisor, volunteer lead, or staff members.

• Demonstrate reliability and commitment to my assigned shift by showing up consistently and on time. Notify WHS as far in advance as possible of all absences, including extended leave or resignation.

• Behave in a professional manner while representing WHS, with the understanding that when I identify myself as a volunteer, my words and conduct are a reflection on the shelter.

• Before using my personal vehicle for shelter business, consult with my insurance carrier to confirm that my insurance provides coverage for this.

• Give permission to be photographed, interviewed or filmed for use in publicizing the work of the Willamette Humane Society. This may be used in newsletters, brochures and other types of public relations items to help our community become more aware of WHS and the work we do on behalf of animals.

• Maintain the confidentiality of information that I encounter in my role as a volunteer. This includes information about members of the public who have surrendered or adopted an animal, donors, and volunteers.

If I am the parent or legal guardian of a child of a child under eighteen years old who is applying to volunteer through this online questionnaire, I hereby give consent for my child or ward, as the case may be, to become a volunteer for Willamette Humane Society as described in the above volunteer agreement and by checking below join in and agree to be bound by the terms and conditions identified in this agreement.

I hereby, for myself, (or on behalf of my child if he/she is a minor volunteering for the Willamette Humane Society) my heirs, my executors and administrators, remise, release and discharge Willamette Humane Society, its officers and employees form all claims, demands, actions, or causes of action, on account of any injury to which may occur during the care of any animal, and/or while involved in any activity as a volunteer for Willamette Humane Society.
