Terms and Conditions of Participation

Please read our hold harmless liability waiver and photography agreement below. Selecting "I Agree" and submission of this form constitutes your agreement to the organizational policies and waiver in its entirety.

This is a voluntary release of liability and a complete assumption of risk. I hereby release Okinawa Leadership Seminar (hereinafter “OLS”) and its officers, Marine Corps Base Camp Smedley Butler, the United States Marine Corps, the Department of the Navy, the United States Government, and all agencies and instrumentalities thereof, its agents, officers, servants, and personnel (hereinafter “the government”), from any and all liability, claims, demands and actions whatsoever resulting from my presence on Camp Foster, MCB Butler, or any other US military installation on Okinawa, or my involvement in the Okinawa Leadership Seminar.

This release applies to myself, and to my parents, spouse, children, guardian, executors, future heirs, assigns, creditors, and administrators. This release of liability includes, but is not limited to claims based on negligence, both passive and active, of OLS and the government arising out of, or relating to any loss, damage, illness, death, or injury that may be sustained while on MCB Butler and/or all other US military installations on Okinawa. This release also applies to all dangers inherently involved in the event in which I desire to participate. I understand that the risks involved in this activity include, but are not limited to, slips, food poisoning, my personal physical condition, vehicles, weather, exposure to the elements, other participants, and lack of hydration.

Known risks aboard military installations include, but are not limited to: (1) Injuries or death resulting from strenuous activities; (2) Injuries or death resulting from recreational activities; (3) High volumes of traffic by civilian and military vehicles; (4) Interactions with animals, both wild and domestic; (5) Significant distances from training and recreational areas to medical treatment facilities or hospitals; (6) Potentially hazardous training activities, including but not limited to, range firing, aircraft operations, hi king, land navigation over hazardous terrain, and field maneuvers; and (7) Hazards inherent to firing weapons, including, but not limited to, being wounded by errant projectiles, being injured by the target apparatus, and exploding ammunition or weapons.

I hereby authorize emergency medical treatment in the event of injury or illness. I also authorize trained health care providers, including, but not limited to physicians, nurses, nurse practitioners, and hospital corpsmen, to administer routine and/or emergency medicines and treatments, as needed.

OLS provides information free of charge and informational in nature. Material presented is the property of the presenters and crewmembers, and may not be reproduced in any form without their express written consent. I agree to hold harmless OLS and its volunteers for any liabilities arising from actions I may take in the application or usage of any information presented at OLS.

I grant to OLS, its representatives and volunteers, the right to take photographs and/or video of myself and my property at any OLS-related functions and to use any and all media, now or hereafter known, and exclusively for the purpose of marketing, promoting, and illustrating OLS. I authorize OLS, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I further consent that my name and identity may be revealed therein or by descriptive text and commentary. 

I do hereby release to OLS, its agents and volunteers, all rights to exhibit this work in print and electronic form publicly and privately and to market and sell copies. I waive any rights, claims, or interest I may have to control the use of my identity or likeness in whatever media used.

I understand that there will be no financial or other remuneration for recording me or for any voluntary work I do for OLS.

I represent that I am at least 18 years of age, have read and understand the foregoing Informed Consent, Waiver of Liability, and Photo Release, and am competent to execute this agreement.
