By submitting this application, you are agreeing to the following:

I understand this application does not guarantee a position as a Family volunteer with the OKC Zoo.

I understand all adults 18+ will be subject to a background check.

I understand we will need to purchase uniform shirts. The cost is $15 (short-sleeved) or $20 (long-sleeved) each.

I understand adults will be required to sign a liability waiver, provide emergency contact information, and complete training necessary for each volunteer opportunity.

I agree my family will uphold all Zoo safety regulations, conduct policies, and confidentiality policies.

I understand that the OKC Zoo reserves the right to terminate our families volunteer status at its discretion.

I agree that to be eligible for the program, children must be 8 to 13 years old by May 1, 2021, and they must volunteer with an adult who is a parent or guardian. Max of 2 children per adult.
