2. Have you ever volunteered here or at another location? Please explain.

3. How would you define what it means to be a volunteer?

I understand that, according to the Dayton Society of Natural History's eligibility policy, any volunteer position that might be offered to me is contingent upon the successful completion of any pre-volunteer interviews, orientations, and paperwork.

(under the age of 18) I acknowledge that to be a volunteer my parent/guardian must complete and submit the required parent/guardian approval and photo release forms before my first day of volunteering, and that I cannot begin volunteering until they are submitted.

(ages 18 and up) I authorize a thorough volunteer background investigation and agree to cooperate in such investigations. I agree to release, from all liabilities or responsibilities, all persons, agencies, and corporations requesting or supplying such information.

I understand that volunteer positions at the Dayton Society of Natural History carry no definite period of time and that I may terminate my volunteer relationship with this organization at any time, for any reason, and that this organization has the same right. If engaged as a volunteer, I release the organization from any liability for future references it may provide regarding my volunteer history at this organization.
