TBRHSC is designated for the provision of Health Care Services in French and encourages all current and potential team members to identify their ability to communicate in French, Ojibwe, or other languages.


- I understand that, IF SELECTED to join the TBRHSC volunteer team, my acceptance will be conditional on the the submission of two references from people who know me well (ie: teacher, employer, coach, etc.). I am responsible to give them the reference form and see that it is sent to Volunteer Services. Reference forms are on the TBRHSC website.

- I hereby authorize and release from all liability my present/previous employer and/or educational institutional/volunteer placement to provide Volunteer Services with reference information concerning me, including but not limited to achievement, performance, attendance, employment/educational history, disciplinary information and reason for separation of employment and/or education.

Volunteer Acceptance Guidelines

- I certify that I am 16 years of age or older.
- I certify that information in this application is correct to the best of my knowledge.
- I understand that a false statement may disqualify me from becoming a volunteer, or cause my dismissal.
- I understand that, if selected to join the TBRHSC volunteer team, my acceptance will be conditional on the submission of two references.
- I understand, that I will be requested to submit the results of a Criminal Record Check or Vulnerable Record Check (18+ years old).
- I understand, if accepted, I must complete Health Screening in accordance with the Public Hospitals Act (Ontario).
- I agree to make a regular commitment to TBRHSC for a minimum of 6 months and or a minimum of 50 hours of service.

Note: This application does not guarantee a placement at TBRHSC.

We look forward to receiving your application; however, due to overwhelming support from our community, not all applicants will be contacted for an interview. Your application will be kept on file for six months.
