By submitting this form, I attest that the information I have provided on this form is true and accurate. If a youth volunteer (under 18 years old), I have received the consent of my parent/guardian(s) and will be accompanied by an adult when volunteering at the Shelter.
All volunteers are required to wear a mask over their nose and mouth while volunteering. This requirement was recommended by Boulder County Public Health as a way to moderate the spread of Covid-19. We also encourage Shelter residents to wear masks, but it is not required.
I understand and agree that my participation as a volunteer is completely voluntary. As a volunteer, I do not expect to receive, and understand that I will not receive, any salary, compensation, or payment of any kind whatsoever from the Boulder Shelter for the Homeless. I also understand and agree that my participation as a volunteer does not entitle me to any employment type benefits, including but not limited to employment insurance programs, workers' compensation benefits in any form, vacations or personal leave or sick time.
I understand and acknowledge that my participation as a volunteer at the Boulder Shelter for the Homeless exposes me to a risk of personal injury, including serious personal injury or death, and loss or damage to personal property. I hereby agree to release the individual members of the Boulder Shelter for the Homeless staff and Board of Directors, and hold harmless the above persons and organization, against any and all damages, claims, demands, rights, and causes of action of whatever kind and nature, based upon injuries or property damages to, or death of, any person (including without limitation any employee, invitee, resident, or other person), arising out of the use of the facilities on the property or services provided therein or thereon, whether or not caused by any negligence of the Boulder Shelter for the Homeless staff or Board of Directors.
I agree to familiarize myself with Boulder Shelter for the Homeless policies, procedures, and volunteer guidelines and to comply with both the letter and the spirit of these policies, procedures, and guidelines. I fully understand that the Boulder Shelter for the Homeless expects high standards of moral and ethical treatment of any persons placed under its care.
In the event that I am exposed to client information, including but not limited to name, age and mental health status, I understand and agree that such information is strictly confidential. I agree to abide by the Boulder Shelter for the Homeless confidentiality policy and will not confirm the presence or absence of any specific individuals who use Shelter services. I also will not reveal the name or any other identifying information of any Shelter resident to anyone outside of the Shelter.
I fully understand and agree that the Boulder Shelter for the Homeless, at its sole discretion, may terminate my participation as a volunteer at any time and for any reason.
I also grant permission to the Boulder Shelter for the Homeless to use photographs taken of me while volunteering on its website or in other official printed publications without further consideration, and I acknowledge the Shelter's right to crop or treat the photographs at its discretion.
