Children between 10 and 15 can volunteer with a parent or legal guardian volunteer who has signed up and attended the volunteer orientation. This is not an application for employment. Terms of Acknowledgment and Volunteer Waiver

The applicant agrees all above information given is correct and current, and that giving misleading or incorrect information may be subject to suspension or termination. CBHS agrees not to share or sell personal information.

I agree to comply with all of the rules and regulations established by CBHS. I understand that failure to do so may result in my immediate termination as a volunteer.

I acknowledge that my services are provided strictly on a volunteer basis, without any pay or compensation of any kind, and without liability of any nature on behalf of CBHS, all services to be performed by me at my own risk.

I recognize that in handling animals and performing other volunteer tasks, exists a risk of injury, including physical harm, caused by the animals.

I understand that public education is an important function of CBHS and I will allow CBHS to use any photographs taken of me to positively benefit CBHS.

On behalf of myself, my heirs, personal representatives, and executors, I hereby release, discharge, indemnify, and hold harmless CBHS, its agents, servants, and employees from any and all claims, causes of action, or demands, of any nature and cause, including costs and attorney’s fees incurred by CBHS in connection with my services for CBHS, including, but not limited to animal bites, accidents, or injuries.

Volunteers under the age of 18 will need a physical copy of the waiver signed by a parent or guardian and brought to orientation. A copy of the volunteer waiver can be found on CedarBendHumane.org under the Volunteer section.
