Volunteers 18 and older are required to provide copies of the PA Act 34 Criminal Record Check and the PA Act 33 Child Abuse History Clearance. Volunteers over 18 are also required to provide an FBI Criminal Background Check or, if having lived in Pennsylvania continuously for the last 10 years, sign a Disclosure Statement Application for Volunteers. The Disclosure Form will be provided, if applicable, at the Initial Orientation and Interview. Clearances are valid for 5 years from the date of issuance. Step-by-step instructions on how to apply for all clearances will accompany the welcome letter that will be issued upon receipt of your application.
In lieu of clearances, volunteers under the age of 18 must submit a letter of permission to volunteer from a parent or guardian and a letter of recommendation from a supervisory adult, such as a school or community official, who can speak to the volunteer applicant's character and abilities.
