Disclosure - Medical

Do you have any medical conditions that may limit your physical activity at the HSEP?

Disclosure - Other

Have you ever been convicted of or pled guilty to any type of crime or offense other than a minor traffic violation? A conviction record will not necessarily disqualify a volunteer, as various factors will be taken into consideration. Answer YES or NO. If YES, also provide Date, Place, Court, and Type of Offense.

Social Media & Marketing

The skill items you list below are simply to give shelter personnel a better idea of what volunteer opportunities you might be most interested in. This has no effect on your acceptance into the HSEP Volunteer Program.
