Please note that a conviction does not necessary preclude you from volunteering, this is a confidential application and any information you provide is used for the sole purpose of the volunteer screening process. Agency Agreements

In order to participate as a volunteer at Operation HOPE-North County, your agreement to the following items is required: 1) I understand and agree that by submitting this application, I am authorizing Operation HOPE-North County to make inquiries concerning my suitability as a volunteer, including a check for any past criminal record. 2) I will participate in a total of 2 training hours. 3) I understand that the information regarding all cases is confidential and may not be discussed outside the agency. 4) Any breach in confidentiality will result in immediate termination from the program.

Consumer Report Copy

We will be obtaining a criminal history report from Intellicorp. You have the right to request from them, upon proper identification, the nature and substance of all information in its files on you, including the sources of information, and the recipients of any reports on you, which the agency has previously furnished within the three-year period preceding your request. You may view their file maintained on you by Intellicorp during normal business hours. You may also obtain a copy of their file upon submitting proper identification and paying the costs of duplication services. Upon making a written request, you may receive a summary of your report via telephone. Check the box below if you wish to receive a copy.

I understand that I am working at all times on a volunteer basis, without monetary compensation or benefits, and not paid as an employee. As an Operation HOPE-North County volunteer, I give Operation HOPE-North County permission to use any photographs or videos made of me during my service without obligation or compensation to me. I understand that Operation HOPE-North County reserves the right to end my volunteer services at any time.
